When the time comes, your family and friends will also find comfort in knowing that your funeral is carried out just the way you would want it.

You can fill as many sections as you want, and remember, you can always revisit the form later if you want to change or add anything. Once you have completed your form, we will email you a copy, and we will also keep a copy for you. Please tell someone close to you where it is, or perhaps even let them look after the form for you.
